We provide case management services for Medicaid Waiver in West Virginia

We also provide case management services to private individuals.

One of the formidable challenges in the new century is to provide health care to the rapidly increasing number of aging persons throughout the world.  Health Consultants Plus Inc., believes without question, the home is the desired setting for receiving care.

Preserving independence and improving functional abilities are central to the Medicaid Waiver Home and Community Based Service Program, and to private individuals.  The program is best defined as an array of health and social support services to clients in their own home.  Such coordinated services may prevent, delay, or be a substitute for temporary long term institutional care.

Medicaid Waiver Home and Community Based Case Management Services include:

1. Case Management

   1. Screening, assessment, and care planning

   2. Arranging for service provision

   3. Monitoring and reassessment

   4. Client advocacy

2. Homemaker services (provided by community agencies certified by the West Virginia Department of Health and Human    Resources)

1. Money management

2. Nutrition, meal preparation, and dishwashing

3. Personal care

4. Social and emotional support

5. Communication and ambulation assistance

6. Housekeeping and cleaning

7. Teaching safety techniques

8. Other services as defined in the plan of care

Health Consultants Plus, Inc.'s highly skilled case management team is there to guide you every step of the way, from screening, and assessing, to developing a plan of care.  Our team consults with relatives , friends, and physicians, in the elderly or disabled person's life, building a picture of needs, whether physical, emotional, social, or economic.

Together, they make a decision about how to maximize the individual's independence and set about the task of coordinating all available resources to meet the agreed goal.  Health Consultants Plus, Inc. services the following counties in West Virginia with Medicaid Waiver Home and Community Based Services Program:

Doddridge, Harrison, Lewis, Marion, Pleasants, Ritchie, Taylor, and Wood.

Services are available to persons who meet the following criteria:

• 18 years of age or older

• A resident in one of the counties we currently serve

• Medically in need of nursing home care, as determined by the West Virginia Office of Medical Services

• Willing participant in the program

• Determined appropriate for home and community based care by Health Consultants Plus, Inc. case management team

• Require services which do not exceed the statewide average cost of nursing home care

• Financially eligible for Medicaid under special guidelines, as determined by economic service workers of the WV Department of Health and Human Resources
